Firing Zbrush, i´d say you start with a sphere, but you actually starts with you SCALE MODEL. This is so all your actors are the same size on your scenes, else you end up with giants or ant-men when loading pre-made actors (sounds silly, but rescalling a rigged character IS a nightmare - you better off doing everything from the bottom again).
