When did cooking manuals/cookbooks start including actual measurements? I've got a printing of the Boston Cooking-School Cookbook from 1909 and they kind of assume you know what you're doing enough because there are no proportions/measurements

Why does the milk in my refrigerator keep becoming chunky before it expires?

I usually buy a gallon of 2% to last me two weeks, but the milk sometimes turns to chunks before the end of the second week. The refrigerator is kept at 40 F, which I thought was low enough, but I'm worried that the milk keeps doing this because it's not kept cold enough.
