I imagine that you actually act like Tomoko in real life, I did too until I turned 18 and had to be around people on a daily basis. (was an actual hikki throughout hs and got GED)
I wear shirts the range from Star Wars to Dragon Ball and even Berserk IRL on a normal basis and just try to ignore other people around me. Once you get used to it it's not that bad

Go at lunch rush and try to sit at the bar. Everyone's going to be busy but you'll be visible just enough to know that you need to order or to be left alone. They will keep social interaction to a minimum because they'll be too busy making rolls. You gotta remember a lot of that jap shit was created for socially awkward people.

Let me tell something about people working customer service gigs. They barely see you. You are a human shaped blob that orders food and waves their credit card st the thing when they're done. I work at a uni in ID card department. Tranny rolled through with a card. Seemed to be embarrassed that his picture was him dressed like a man and here we was standing at my counter in a dress. What did I care about this? Just show me your papers and say the correct words so i can give you you're card and set up your residence door access. Papers proffered, correct answers given, here is card, take this faq brochure then, have a nice day. At restaurant i assume it's state you're order, don't inconvenience them with weird requests, eat, pay, probably tip (i dunno i never worked as a waiter), get out so the next blob can give order/eat/pay.

Don't be embarrassed in this situation. It's not warranted.
