If you're West of the rocky mountains, go for an In-N-Out. Fatburger is good too, but it costs more than In-N-Out. Chipotle is probably the best burrito you can get from a ubiquitous fast food place (although visit a Cafe Rio if you're lucky enough to find one).

The only places I would go to on that list are Starbucks (if there were no other source for espresso and I wanted one) and In-N-Out (once every few years). Maybe a bean burrito at Taco Bell if I were hungry and the only other options were other fast food chains.

From your list (and I've only tried a tiny portion, I have only been to the US a couple time), I'd disregard most "real fast food" places (Wendy's wasn't bad tho) and focus on "Roadside, America" food, my own private Bill Bryson-esque expectation of the US.

That'd be Denny's, Johnnie Rockets and IHOP.
Starbucks for your caffeine fix (I don't see a coffee bean and tea leaf or Costa Cafe in your list) and Krispy Kreme for guilty pleasures.

The only time I ever visit Jack in the Box is if it's 1AM and my sober friend is driving me and my drunk friends around. Absolutely. I recommend Intellegentsia, but I have yet to be disappointed by a non-chain coffee shop in NYC.

Dairy Queen is pretty good, their ice cream is some of my favorite soft serve. I think it depends on your tastes. I wouldn't compare it to Baskin Robins or Cold Stone because those specialize in hard ice cream and DQ is all soft-serve based shit.

I think you can find something better in both NY and LA. I personally enjoy Amorino Gelato near Washington Square Park in NYC but if you do a search you can find a number of local ice cream places all around Manhattan and such.
