You MUST tell them to make it fresh. You'll have to wait 5 minutes, but there is no comparison between a flayo where the fish just came out of the fryer and is very crispy and will burn your mouth with both scolding oil from the batter and piping hot steam from the fish, and one that's spent even a minute in the plastic heating tray.

I worked there and the fillet-o-fish is fried in relatively fresher and cleaner oil, because the vat is only used for fish, which is rarely ordered. So getting one cooked to order is probably as good as it gets at mcdonalds.

Fair. I live in a flyover where the wagies are mostly older white people - most likely widowers who are a few years short of being able to collect their partner's pension. So basic requests are fulfilled.

The ones where I lived in Texas were all immigrants and they always go the orders right, but short circuited anytime there was something custom so I just stopped.
