I don't follow what this guy does mostly, but as someone who is a director of marketing for one of the world's top global agencies, I envy him and think he's actually just putting on an act to make millions. I don't think he's specifically mentally ill. I'd do what he's doing for the money (but would cut it off at a point).

He might pump it up for the cameras but all that shit has some truth to it, it can't not be. Nobody can lie and act 24/7 365. It's not hard to tell he has issues. His relationship makes no sense and is clearly bad for the both of them but leaving is accepting that it's not going well, followed by pain, which you need to improve upon yourself to actually move on from, so they never actually stick to it. I'd say he hates himself and is a narcissist too.
