It's made separately.
At home just use a single thicker patty. You're not interested in patties so thin that the lack of moisture makes them shelf stable. You're interested in a juicy burger.

in my experience, the middle bun is the slipping point where the burger shears appart. The fine crumb of the bun, and the special sauce acting as lubricant cause there to be less friction than two meat patties rubbing together.

There are various sources that claim it has shrunk. It seems to be the prevailing opinion. McDonald's themselves claim otherwise.

The bun has definitely shrunk. It did so in the 70's because the smaller bun made the patty look bigger.

Different regions get different sized patties. So you cant entirely trust still images, because the before and after can come from different regions.

I suspect the burgers have shrunk. The more detailed claims that the size has remained the same are mostly based upon the assertion that pre cooked weight is the same as it used to be.

However supermarkets play that same shonk. They increase the percentage of fat, heat the meat to the temperature where it holds the most brine, then vacuum seal it. This results in you paying for water at the price of meat, and it causes the meat to shrink like crazy when cooked. I suspect the cooked weight is much lower.

You can't trust those, though. They actually put slices of cardboard in between the ingredients in those to "bulk up" the look of the burgers (all fast food chains do this) so comparing a commercial to reality is always boing to have the commercial look larger. This holds true even in modern-day food.

Look at a loaf of white bread.
Notice how the outside of the loaf (the "crust") is much darker than the interior of the loaf?
That's because it was on the outside of what was baked.
Now look at a hamburger bun.
Notice the top and bottom have a crust, but the inside doesn't?
Now look at the middle piece from a Big Mac.
It's not a huge mystery, but obviously, the outside was exposed while baking, but the top and bottom weren't.
The answer to OP's question is "they make an extra tall bun, then slice it into thirds".

Yes. This is how they look when they show up in the store.

If anyone's wondering what the purpose of the middle bun is, it's meant to hold more mac sauce and lettuce on the burger. If you just squirted the sauce between two patties, it would make even more of a mess and wouldn't stay on the burger. They've actually adjusted the ratio between the bun parts a number of times over the years, as well as the amount of sauce the dispenser guns squirt out.
