Umm no sweaty, you're supposed to take it as prescribed (usually daily) and try NOT to drink, it reduces the cravings and if you DO drink the alcohol doesn't have so much of an effect and/or appeal.
It was first used to treat opioid addiction as it blocks those receptors but it works on alcoholics as well.

Naltrexone has essentially no "anti-craving" effects if taken on

Actually, that is a bit of an oversimplification because other
things also release endorphins. Taking naltrexone without drinking may reduce one's interest in sweets or sex a little, but it will
not reduce the craving for alcohol. Extinction only affects those endorphin-mediated behaviors that occur while on the medication.

I am on naltrexone (50-100mg per day, depending on what i feel like). Yeah I still drink, and still enjoy it, I just have 2-5 drinks instead of 9 and don't feel antsy when I stop for the night.

It's a useful drug.

Correct. Big pharma and AA are the authorities on naltrexone. Pharma wants you to buy pills endlessly and so backs AA in the idea that you can never be cured. AA is open to medications but would never consider allowing anyone to take another drink without calling it a relapse.
Doctors dont know much about naltrexone, and I've never talked to a psych or doctor who knew of the sinclair method.
