Alice suffers from regular traumatic breakdowns caused by seeing her entire family get slaughtered in front of her during the events of Mystic Square. Her failure at exacting revenge and the subsequent slavery and harassment by the protagonists only worsened her grip on reality.
On normal days she could usually go about her business without issue. But if Marisa, Yuuka, or particularly Reimu are around or even mentioned things could get bad quickly. Her birthday and anything that reminds her of her sisters and mother are also major trigger points.
It's likely that her dolls are a coping mechanism used to represent the family she lost so brutally so young, and is the only thing that keeps her sane enough to not start a bloody incident herself.

All of this makes it hard for her to maintain any friendships, and why she lives alone secluded in the Forest of Magic playing with dolls.
