Tamaki and Sakiko are biological sisters and their parents are Shion and Satoshi, which explains why one looks like a Hojo and the other a Sonozaki. The reason Shion gave her kids to Mion and Satoko is because she had them before Satoshi actually woke up, not wanting the kids to grow up with a comatose dad, and not knowing how to explain to Satoshi what she did to him during his coma. Satoko was now married and could take care of Sakiko, which no one would question is her daughter, and the same can be said for Mion and Tamaki.
Tamaki is about a year younger than Sakiko because Shion couldn't help herself and accidentally did the same thing twice.
