I have an arcade stick, a dualshock 4 and keyboard, and I honestly prefer the keyboard.
I don't think the stick is very good for doing all those small adjustments I'm used to doing with the keyboard. I could see a dpad working if you claw it so you can tap both up-down and left-right at any moment, but just using my thumb wasn't doing it for me.
If your wrists are killing you, I'd just suggest getting a more ergonomic keyboard, and maybe use software to remap your keys when you play touhou, if those keyboards aren't that ergonomic while using the arrow keys. Pic perhaps related? I haven't tried it or anything, but its probably not a bad idea to get an ergonomic keyboard anyway, to avoid problems later in life.
