Touhou's popularity in the west exploded right around the time tf2 and steam groups exploded, and it was the 'default fps game' for a lot of touhou fans for a span of at least a few years. The social nature of the game at the time, combined with sprays/names/avatars, and the ease of making custom content/modding, probably lent itself well to the prolific nature of 2hu content creators.

That's just my guess based on my own observations anyway; it could be way off, but it sounds about right.

The basic bitch Valve servers don't allow any custom things apart from HUDs, but you can set the server command up to allow different models or textures or whatever

You receive a "check" when you join a server, and the whitelist decides what kind of things are good and which are naughty.
I don't think you can swap them out like a PS1 disc, as I'd imagine the game would force a comparison every time you'd load in a different texture/model/whatever.

Though I wouldn't be surprised if there are hacks out there which do allow you to do things like that
