The mainstream appeal is that the characters act and talk like modern liberal people and "medieval hardcore" stuff he likes to boast about is all just whores and torture and sex and violence and more rape and more sex. Read the Druon series which what asoiaf is derivative of and which is superior. And might peak your interest about history and playing Crusader Kings.
