I've read more heinous shit in online erotica/fapfics, but as far as published works go, Hogg. Honorary mention to that Brazilian(?) necrophile who got kicked out of med school for molesting corpses and somehow managed to get a short story collection published, though I've only read the first story in that. Maybe Dennis Cooper and his obsession with violently murdering twinks could count, but I haven't read any of his stuff yet so can't vouch for him.

It's weird seeing people talking about how fucked up X book is when the level of human suffering that's going on within a 50km radius of where they live is probably beyond their imagination. Most publishers aren't comfortable with putting out something as grim as reality.
