typically the solution is to use a verb for what the persona was doing with the thing they had had. had held, had owned, had used, had been with, that sort of thing. "He had had enough" can be "He had gotten fed up." Sometimes the sentence just needs to be totally changed or removed. But if it's dialogue, I usually keep it in, people say dumb shit like that all the time.

You could always use contractions. especially when writing dialogue, it's a lot more natural (he'd had, she's had ect.). When writing just prose, you can always maneuver around it. Like "He had had to go to the bathroom" could be rewritten as "he felt the need to go to the bathroom"; or, if you are allergic to cliche, "his blatter pressed against his prostate in the most pleasing way."
