I'm reasonably sure it was supposed to be an insect. The translation I used called it a 'bug' I believe. Due to the hard shell making it difficult for him to turn over I can see where people are getting beetle from.

And I thought it was about families who, rather than seeking help for family members who are diseased/crazy just lock them up and pretend they aren't there.
