Those are the 20 100kw electric propulsion units, their NEP vehicle flies 'sideways' while burning with the EP.

The vehicle is configured that way in order to 1) protect the crew compartment from the reactor's radiation, 2) not route high power or fuel lines through the crew compartment and 3) keep the EP exhaust from hitting the vehicle.
