Drugs in general give you withdrawal because they either inhibit the function of receptors on your cells, or activate them. This causes the cell to either generate more of these receptors or remove them in order to bring its average level of activity back to normal, which is why you develop tolerance to drugs. Basically, the cells have created more/less of those receptors, but their activity levels are near normal because of the drugs. When you stop using them cold turkey, the drug is no longer there to compensate for the altered levels of the receptors, and so the behaviour of the cells is thrown out of balance again until they bring the receptor levels back to normal.

I'm sure some smartypants will correct me if I'm wrong but I don't think this has been mapped out fully. Diarrhea symptoms are fairly easy to explain by cAMP hypofunction in intestinal cells, but the rise in temperature is more difficult to explain. If it does in fact occur (instead of being a perceived change of temperature only on the part of the addict) I'd guess it's because opioid receptors are strongly expressed in the hypothalamus which is involved in the regulation of body temperature.

A general rule of thumb is that withdrawal symptoms are the inverse of the physiological effect of the drug itself. Since opioids have a very strong suppressive effect on the sympathetic nervous system by virtue of the opioid receptor distribution, it stands to reason that withdrawal activates the parasympathetic nervous system. Hence, muscle tension and increase in temperature.

Apparently men can also experience spontaneous ejaculation. I have no fucking clue how the hell that could happen though.

That said, that horribly vivid and realistic dream was kinda cool. It almost made up for going through the experience of withdrawal, almost. It was certainly more interesting and mentally intriguing than heroin itself, which is probably one of the most boring drugs known to man. Give me mainline ketamine any day.
