Playing through Doom for the first time on UV and e4m2 is the first mission to make me mad. What is this?

I cleared all the barons. I ended up doing the entire grey/dark area with 19 hp and savescumming to get the blue key to finish the mission.

At least Perfect Hatred is dickish in kind of a unique way with the platforming and tiny ledges. E4M1 is the worst map. Just a small cramped map with little ammo. Complete trash.

They started to get more avant-garde with map design now that vanilla Doom and Doom II were already released.

People had complained that Doom was too easy on UV, which first resulted in Nightmare! being added as a joke difficulty with updates, but it would also lead to an upped ante for Doom 2, and Ultimate Doom, which was the first proper retail release of Doom 1. Ultimate Doom had an added episode, which was designed to be more challenging as well.
Largely though, it's only like the first three maps which actually go really hard, it mellows out a lot after that.

Doom was originally sold only by mail-order, the demo with the first episode shared online for free. When GT Interactive wanted to pay iD to make Doom 2 as a regular retail product, they figured they'd give the first game a retail release too, so people could grab both games off the shelf in a store and have the full experience. They'd call this product The Ultimate Doom.

To incentivize this, they added a fourth episode to the first game, Thy Flesh Consumed, which had 9 new official levels designed to be more challenging than the original game. If you had already bought Doom by mail-order, you could get your old version of Doom upgraded to The Ultimate Doom for free with just a downloadable patch, so you didn't have to buy the game again to get that new bonus content.
