To me, it sounds like Kiara probably has a severe trapezius tear, since it'd especially fit the bill as to why she's been dealing with bad shoulder pain for such a long time. I'm assuming she probably did some sort of physical activity on her vacation that she wasn't properly loosened up for, and when the delayed onset of muscle soreness kicked in, she probably tried to stretch it out and did a gnarly tear as a result.

What likely happened, given an extended timeline, was that when she got seen for her shoulder issue, she was told to go to a chiropractor, and what the chiropractor likely did was merely loosen the tension in her traps. As an actual physiologist, the most important thing to do is to actually make a clean, manual tear in the muscle, give the patient a lot of protein to supplement in their diet, and obviously some quality painkillers (usually some higher-dose or OTC NSAIDs, unless there's several tears, in which case, they will impede recovery too much and we'll just prescribe the other usual suspects). The main issue here is that since she didn't take a permanent route to recovery by seeing a chiropractor, which by the way, NEVER DO, it'll tighten itself up again. She likely did not see a chiropractor in some time (especially given she was gone for a week) and therefore the muscle tightened / cramped up more than it usually did, leading to her probably trying to unkink it subconsciously and then tearing the muscle. My prognosis is that Kiara likely has a very underdeveloped trapezius, which is extremely common in women, especially those who often have inconsistent levels of physical activity while working at a desks for prolonged periods of time.
