I think of things in terms of two abstractions: key points and scenes. Key points are just little succinct explanations of things that I want to happen in the fic, like at a conceptual level. In the context of a weight gain fic, I might wanna have a point where the girl is at 200lbs, 300, etc., or have a point where she breaks a chair, or one where her gluttony has consumed her rationality enough to do something stupid for more food.

Then I break everything down into scenes with clean breaks between them, each of which contains at minimum one key point. The goal should be to build up to each checkpoint in sequence with as much or as little flair as I'm feeling up to, and introductions are no exception. Unless it's just a short drabble covering a single sexy scene, key point #1 is almost always "establish the reason X character is going to be gaining/has already gained some weight". It's amazing how much difference even a tiny bit of effort put into rationalizing an absurd scenario can make regarding a reader's suspension of disbelief and level of horniness.
