It probably varies a little bit each time but it seems like Cover gives the artist a general look and theme they're going for, artist does their interpretation and after a few revisions, some of those revisions getting input from the talent, it's approved and they move forward with it. The talent are definitely not in control of the process though. Fundamentally, Cover wants to make sure the character is marketable and if their chosen talent doesn't want to do the character, they'll find someone who does.
