They've messed up a couple times but it's never a big deal. And I'm speaking purely for EN. I'm sure they know each other's real names just from talking to each other in private but I'm sure when they're together on business they stick to the fake names even off camera.

This. I have a small group of about seven people I met on the internet that I've been friends with for over two decades. When I visit them or they visit me, we have to make a conscious effort not to use our online handles so our families/IRL friends aren't confused.

I think this works under the assumption that they have actual name-like names. I have some friends who I knew online for years before ever meeting them in person (and learning their real name), and it's very difficult for me to refer to them by their name. It just feels wrong. Whether I'm talking in person, over voice chat, or even just thinking about them, they're their screenname. I imagine holo and other vtuber groups are similar.
The case I find funny is when a few of them go out somewhere as a group and they need a second level of codenames to refer to each other in public
