If you're going to do something that doesn't really fit with the setting (like a vtuber in a medieval kingdom), it's best to use Story rather than Do or Say, since you can write the environment to be whatever you want. Also, just a personal thing, but I'd recommend using the Custom prompt rather than one of the prebuilt ones, since the AI can get a bit railroadish in generic settings.

Depends on how much you use it. There's definitely an improvement in both content and writing that the AI produces when you use Dragon. I use AID for multiple stories every week, so it's good value for me. You definitely don't need Dragon to have a good story, but it does help.
